All Memberships based on Monthly checking autodebit or yearly prepay only.  All memberships are non-transferable.  All services must be used within 60 days. NO REFUNDS.  Massage and chiropractic adjustments are for wellness care (non-symptomatic) and is not applicable for injuries, auto accidents, or work related conditions.  Each “half hour” is based on twenty-five minute increments and each “hour” is based on fifty-five minute increments.

Rub Club Membership Guidelines

1. 1.All memberships are based on monthly debit or annual prepayment.
   

2. 2.All massages are to be used within 60 days of previous months debit date.  Those not used will be forfeited.
   

3. 3.All Massage visits are based on availability, hours and discretion of the doctor.
   

4. 4.All memberships are non-transferable.
   

5. 5.The Rub Club reserves the right to cancel client’s membership at any time without cause.
   

6. 6.All Rub Club memberships are for symptom free maintenance care and can not be used for treatment of symptoms as the result of auto accident, work injury, or any other acute injury.
   

7. 7.All membership use is based on appointment and hours of operation of the Rub Club.
   

8. 8.No cash or credit refunds for unused services.
